Eric L. Middlebrooks, 49 of Toledo, OH, died Wednesday, June 1, 2016, in St. Anne Mercy Hospital. Born March 7, 1967, in Toledo, OH, he was the son of Jerry and Esther (Church) Middlebrooks.
Eric is survived by his siblings, Charles (Michelle) Middlebrooks, Darlene (Dan) Schetter and Pam (Keith) Wires. He was preceded in death by his parents and brother, Matthew Middlebrooks.
Visitation will be from 1-5 pm, Saturday, June 4, 2016, at the Michael W. Pawlak Funeral Home, 1640 Smith Rd. (at Jackman) Temperance, MI, where a memorial service will follow at 5:00 pm. The family wishes to extend a special thanks to his primary care giver for over 20 years, Vickki Brogan, and the entire staff at Anne Grady Center.
